Thank you for booking your class to complete your class registration, please fill out this form and complete payment. You will receive a confirmation email from Lynsie within 48 hours to confirm your payment was received and your spot is reserved. To hold your spot pre-payment is required. You can mail payment by check (to Ideal DOG 3450 Senn Road Omro WI 54963), or pay online: paypal.me/IdealDOGLLC OR complete a paper form in person at The Pampurr'd Pet (hours Mon-Sat). You will receive an email if the class you want is full and be added to the wait list. Payment at The Pampurr'd Pet must be cash or check to Ideal DOG only- no credit cards. For Puppy Play Date registration, just complete the form and write" Puppy Play Date" as the class description. Play Date payments can be made at the door( $10 cash)

Ideal DOG -DOG TRAINING SERVICES AGREEMENT Classes and lessons are filled on a first-come, first-served basis. A spot will not be held until payment is received along with signed Training Agreement. These items must be received prior to commencement of first class. By clicking on the box/signing you are agreeing to this and stating your dog is current on vaccinations. SERVICES PROVIDED: Classes are limited to no more than 8 dogs so that personal attention may be given to each dog-handler team. Trainer, at his/her discretion, may refuse entry to any dog which is not healthy, is aggressive or does not seem suitable for the services provided. PAYMENT: Payment is due prior to the start of training. No refunds will be given after class has commenced. Registration form/check payment can be mailed to Ideal DOG 3450 Senn Road, Omro, WI 54963 or payment online via PayPal REMOVAL FROM TRAINING: Trainer, at his/her sole discretion, may remove a pet from training if a hazard or threat of any nature to any other animal or person is present. No refund will be given under such circumstances. CLEANLINESS: Owner is responsible for the removal of excrement that their pet is responsible for both inside and outside. LIABILITY: Owner agrees to indemnify and hold Trainer and Trainer’s employees/agents harmless from all liability for any loss, damage or injury to persons, animals or property arising from or related to Owner’s pet. Owner agrees that Trainer shall not be liable for loss or damage to animal for any reason unless said loss or damage was a direct result of Trainer’s negligence. I hereby grant Ideal DOG LLC permission to use my likeness in a photograph, video, or other digital media in any and all of its publications, including web-based publications *
